Abstract


Peptide therapy sits at an uncomfortable intersection: enormous therapeutic potential meets categorical regulatory ambiguity meets minimal longitudinal safety data meets widespread clinician enthusiasm unbounded by training. This is precisely the clinical context where AI decision support promises maximal value — and maximal harm.

The promise is obvious: peptides involve complex pharmacokinetics, patient-specific dosing curves, multi-biomarker interaction profiles, and a clinical knowledge base that evolves faster than human practitioners can track. AI could, in theory, assist with all of it — dosing optimization, contraindication checking, biomarker correlation, adverse event prediction.

The risk is equally obvious: an AI system trained on incomplete data, deployed without rigorous guardrails, and marketed to clinicians with minimal peptide-specific training is not a clinical decision support tool. It is a liability engine.

2.1 The Five Design Principles of HITL in Clinical AI

Principle 1: No Autonomous Clinical Actions

The AI does not prescribe. It does not order. It does not modify. It suggests — and the suggestion must be presented in a format that makes the physician’s approval an active, conscious decision rather than a passive default.

This is not fearmongering. It is pattern recognition. Every high-growth, under-regulated healthcare market eventually faces a regulatory correction. The operators who anticipated that correction and built accordingly capture the post-correction market. The operators who optimized for the pre-correction environment do not.

4.3 The Ethical Case Is Also the Commercial Case

There is a more fundamental argument. We are physicians. We were trained to a professional standard that places patient welfare above commercial optimization. An AI tool that increases prescription volume at the cost of patient safety is not a clinical innovation — it is a violation of our fiduciary duty to patients.

The market will eventually price integrity correctly. V. Conclusion: Governance as Product, Not Constraint

The conventional framing treats governance as overhead — a necessary evil imposed by regulators, lawyers, and risk managers that makes products slower, more expensive, and less competitive. This framing is wrong.

In clinical AI, governance is the product.
